How can I access my requisition forms?

You can access your requisition form/s through the Molecular You - Client Portal.

After you have received and registered your collection kit, you will be prompted to choose a collection site from our designated list. Following this, your requisition form/s will be emailed to you.

Please ensure you print and bring your requisition form/s with you when you provide a sample.

If you need to change your collection site, you can do so in the Molecular You - Client Portal.
